Requesting Your Healthcare Records
Patients may request a copy of their healthcare records by emailing Frontdesk@premierhealthchiropractic.com.
Please include:
- Full name
- Date of birth
- A description of the records being requested
- The best telephone number to reach you if additional information is needed
Our office will provide any additional authorization or documentation required to process the request. Medical record requests will be processed in accordance with applicable Texas and federal laws.
